The Healthy Care Pregnancy Program offers safe and confidential support to individuals who are pregnant or up to seven months post-partum, and have a history of using substances or currently using them.
You can access the Healthy Care Pregnancy Program anytime during your pregnancy or post-partum. We will support you regardless of whether you are living with your baby or currently using substances.
Referrals can be made (with permission of the potential participant) by phone call, text, or email. Referrals can come from other professionals, friends/family or by self-referral.
Once you access our services with our Health Care Pregnancy Program In-reach Worker, we will:
- Refer and connect you to other community supports, medical care and services
- Help you navigate the health care system
- Accompany you to appointments
- Support you through transitions in-and-out of hospital and/or bed-based programs
- Advocate for you with MCFD or a Delegated Agency
- Ensure your basic needs are being met (food, shelter, healthcare)
- Support you in living free from violence (transition houses, safety-planning, legal services).
